I hereby give Drift Dance School permission to use any still and/or moving image being video footage, photographs and/or frames depicting the student named above, whilst performing with Drift Dance School for any of the following uses: Advertisements, marketing, leaflets, or any other use such as for training, educational or publicity purposes.

• I, the legal parent/guardian accept and understand that appropriate physical contact may be necessary within the dance class and hereby give permission for any teacher from Drift Dance School to do so. Any person teaching for Drift Dance School is required to undergo a Disclosure and Barring Service (formally CRB) check.

• Some classes have waiting lists if they are full or by invitation only. We constantly update if a place becomes free or if we see high potential for the elite classes. Please respect these decisions.

• I understand teachers cannot be responsible for my son/daughter after class and that they will not leave the Building until somebody or myself known to them has collected them.

• The information provided in this form will be treated confidentially and will only be used for administration purposes by Drift Dance School. No information will be released to any external parties. To coincide with the changing data protection laws, by agreeing below you are agreeing to Drift Dance School being able to contact you by email and text message in accordance to receiving information, emergency contacts and newsletters regarding school events. All data will be treated with respect and follow the GDPR law process.

Your personal details are required in order to contact you and, if applicable, to do business with you. This forms the lawful basis under which we collect this information; without it we cannot get in touch with you or do business with you.
